Procedures for Professors:
  1. Students will complete their section of the accommodation request form for each test/exam they want to write in the AccessAbility Test Centre and give to you as soon as test/exam is announced or at least one week or 5 school days prior to the scheduled test/exam date.

  2. Professors complete their section of the form:
    1. Include your name, course name and phone number where you can be reached during school hours in case of a problem or discrepancy.
    2. Indicate time allotted for students in class to write the test. Students writing in the ATC are generally granted time and a half, unless otherwise specified on their green sheets.
    3. Indicate reference materials allowed, such as calculators, rulers, dictionaries, notes, etc.
    4. Indicate whether you will pick up completed tests in F2001 or need to have them mailed to your division.
    5. Attach paper, exam booklets, Scantron sheets, etc. as required for the test.

  3. Please deliver the completed Test Accommodation Request form (blue sheet) and test/exam to F2001 at least 5 school days prior to test/exam date or email the on-line interactive Test Accommodation Request form (be sure to save the interactive PDF with students’ names on your desktop prior to emailing) accompanied by the test/exam to aatestcentre@fanshawec.ca or, if afterhours, deliver to the afterhours mail slot at Counselling & Accessibility Services outside of F2010.

  4. If an FOL or other on-line test/exam, please be sure to set-up with the appropriate accommodated time stated on students’ green sheets
