Outlined below are the Group Insurance Benefit premium increases and Sun Life Group Extended Health Care Plans - change of coverage for Vaccines, effective February 1, 2004.

As a reminder, please ensure that you communicate your new Sun Life Certificate Number (provided to you in your last pay statement for 2003) to any service providers that electronically submit your claims, i.e. dentist.
