Compile your disability documentation

When filling out the registration form in step 2, you will be asked to submit disability documentation. You can get started preparing this information ahead of time!

For students registering with Accessibility Services

Please have your regulated health professional complete the Confidential Disability Information Form to confirm your disability impacts. Accessibility Services uses this information to help inform us of your eligibility for specific academic accommodations and supports. 

You may also submit a full psycho-educational assessment report in lieu of a Confidential Disability Information Form if it confirms a learning or other disability. 

 

Don’t have this documentation yet?

That’s okay. If you have other documentation (such as an IEP, medical notes, etc.) or no documentation and you have a disability or suspected disability, you can choose to skip to Step 2.

You can still meet with an Accessibility Advisor to discuss your situation, and they can advise you on your options for obtaining the necessary documentation for us to support you.

Your Advisor may ask you to complete a series of screening questionnaires to help determine whether interim or temporary accommodations can be considered while you work on obtaining the necessary documentation. 

 

For students applying to OSAP

Please have your regulated health professional complete the OSAP Disability Verification Form if you are applying to OSAP and want to be considered for additional grant and bursary funding for disability-related equipment and services. 

This form can be submitted with your online OSAP application or directly to the Financial Aid Department.

Choose your Registration Option and Submit your Form

There are two ways to register with accessibility services. Please review both the options below and choose the one that works best for you.

 

Option 1: Advisor-Guided Registration

This option includes a 50-minute registration appointment with an assigned Accessibility Advisor.

During the meeting, you’ll:

  • Talk about your disability and academic impacts
  • Review your documentation
  • Collaborate on your academic accommodation plan
  • Discuss your OSAP bursary needs and complete application if needed
  • Learn about college services that can support you
  • Ask questions and discuss any concerns

Your accommodation plan will be created after the appointment.

Wait times:

Wait times for an appointment and accommodation plan can range from approximately 3 days to 2 weeks, but this can vary depending on the time of the year. During busy periods, wait times may be longer. 

Option 2: Self-Guided Registration

This option is best if you have the necessary documentation and do not need a registration appointment with your Accessibility Advisor.

If your registration form and documentation are complete, you may receive your accommodation plan within approximately 3-5 business days.

Once submitted:

  • You’ll be assigned an Accessibility Advisor
  • Your Advisor may email you with questions or request a meeting if needed
  • Your Advisor will e-mail you when your Accommodation Letter is ready for you to review and approve online
  • You can book follow-up appointments at any time during your studies
  • Submit your registration form

 

When you’re ready to complete your Registration Form

  1. Log in FanshaweOnline.ca.
  2. Click the College Services menu item
  3. Select Accessibility Online Services.
  4. Click “Registration Form” and follow the instructions.

     

September is our busiest month for student appointments. Waiting until September to register can mean waiting several weeks before you can meet with your Accessibility Advisor to set up your accommodations and services.

Check your Fanshawe email for next steps

Please allow up to three business days for us to respond after you submit your registration form to let you know which Accessibility Advisor you have been assigned to and their contact information. 

What you’ll receive depends on the registration option you chose:

  • Advisor-Guided Registration: you’ll receive an email about how to book your registration appointment with an Accessibility Advisor.
  • Self-Guided Registration: Once you are notified who your Accessibility Advisor is, they will have approximately 3 business days to email you with details about your accommodation plan and how to share your accommodation letters with your professors.

If your registration form or documentation is incomplete, we’ll contact you to explain your options and help you with next steps.

Frequently Asked Questions

Why do I need to provide documentation? 

Fanshawe’s Policy A101  requires documentation for disability-related academic accommodations. This helps us understand how your disability may affect you in a college learning environment and how we can best support you.

Providing documentation is not about proving your disability. It helps us understand the functional impacts you experience in your courses so we can work with you to create appropriate accommodations.

All documentation is reviewed confidentially and with respect for your privacy.

 How does Accessibility Services use the documentation?

Your documentation helps us:

  • Confirm eligibility for Accessibility Services
  • Understand how your disability affects learning in an academic setting
  • Determine appropriate academic accommodations

Accommodations are based on functional impacts, not just a diagnosis. This means:

  • A diagnosis alone does not automatically result in accommodations
  • High school accommodations are not automatically applied in college
  • Accommodation recommendations must be appropriate for a post-secondary learning environment
