Application for Changes of an F Grade formComplete this form to appeal for the change of an 'F' Grade. Students must have reasonable grounds for an appeal such as medical/legal reasons, compassionate reasons, personal cause (e.g., tragic, personal event), procedural error or any other unfair treatment. F grade(s) must be assigned to you at the time of application submission. You cannot submit an application for an anticipated F grade.

Course Credit Transfer FormComplete this form to apply for internal or external course credit. Internal credit is for previous courses completed at Fanshawe College. External credit is for previous courses completed at another institution.

Credit Balance Refund RequestComplete this form to make a request to the Fees Department to initiate a refund for a credit balance on your account. Please note, refunds are provided to international students only in their graduating semester. Learn more about refunds
Credential Request FormComplete this form to request a printed hard copy or duplicate copy of your credential (diploma, degree or certificate). Please note there is a $50 charge for duplicate copies.

Domestic Document Request FormComplete this form if you are a Domestic student requiring a document. Documents include Verification of Enrolment, Program Completion Letter, Confirmation of Admission, and the completion of any third party forms.  
International Document RequestComplete this form if you are an International student requiring a document. Documents include Verification of Enrolment, Program Completion Letter, Placement Letter, Break Letter or English Language Institution Confirmation.  
FANCard Exception Request FormComplete this form to request a FANCard as a fully online student. Learn more about FANCard

Notice of Appeal FormComplete this form to appeal academic decisions relating to grades. Students must have reasonable grounds for an appeal such as merit of work, medical/legal reasons, compassionate reasons, procedural error or any other unfair treatment.
  • Include a letter outlining the circumstances that occurred resulting in the need for the appeal. 
  • Supporting documentation such as emails, doctors note or medical form

Fee Appeal FormComplete this form to appeal a College fee decision. Students must have reasonable grounds for an appeal such as medical/legal reasons, compassionate reasons, personal cause (e.g., tragic, personal event), procedural error or any other unfair treatment.

Supporting documentation for your appeal:

  • Include a letter outlining the circumstances that occurred resulting in the need for the appeal.
  • If you are appealing for a medical reason, you will be asked to provide a signed document from your physician
  • Download the medical form
    If you appeal for a personal reason, you will be asked to provide documentation based on the nature of the event
  • If you appeal for a college error, you will be asked for documentation relevant to the event

Personal Information Change RequestComplete this form to make changes to your legal name (first, middle or surname), your Chosen Name, Sex or Date of Birth.  Please note, documentation is required to change your legal name or date of birth.
  • Student login If legal name change, a copy of government-issued identification is required (ie. Driver’s license or Passport)

Re-admission - Program Transfer​Complete this form to request to be re-admitted to the same program (Level 1) or to request to re-apply to the same program or transfer to another program beyond level one (Level 2 only). Please note: if you wish to change programs or apply to multiple programs you will need to create a new application at www.ontariocolleges.ca.  
Release of Information formComplete this form to grant permission to a 3rd party (parents, spouse, etc.) to access your personal student information such as grades, fees, etc.  
Post-secondary withdrawal formComplete this form to withdraw from your post-secondary programs and all courses. If you withdraw, you will be withdrawn from all of your classes for the term. You will not continue to complete coursework for the term. Learn more about withdrawing
